Become a Resident Navigator

The navigator guides those aging out of foster care who become residents of the Wholistic Transformation campus towards developing and meeting their goals through resources from vetted community agencies and healthcare providers where applicable. The Navigator will help remove obstacles so that clients can achieve their chosen direction by assisting each resident in developing their unique skills and talents, health and spiritual growth to gain stability for success after leaving the program.


There is much work to be done before we bring participants on to ensure we are able to adequately serve them. This work includes:


  • Meet with the network of local human services organizations and survey them regarding program curriculum design, development of an individualized plan (format), identifying MOU-based referral systems for youth, and starting an alliance for young adults who want to create an individualized goal-oriented plan for their future in a peer-to-peer support network.


  • Develop the program curriculum to include the larger foster care community.


  • Develop an outreach plan and application for youth who will enroll in the program and who are interested in becoming residents at the WT campus, including a lease, property rules, and a cooperative maintenance plan as companion documents for the individualized plan(s).


  • Develop a series of databases for applications, tracking client progress and credit earnings, agency networks and referrals, and property maintenance.


  • Begin vetting of the first participants as the "founding" cohort of WT residents.


  • Assist with the development of policies and procedures for the Wholistic Transformation operations including the application process, grievance procedures, safety protocols, mentoring opportunities, relationship commitments, facility repairs, and the roles and responsibilities of staff, the Resident Navigator and the program participants.


  • Prepare the move-in kits and onboarding for lease up with an attached calendar for regular check-ins and feedback.


  • Help the WT Team prepare the property and promote the program.

DUTIES AND RESPONSIBILITIES ONCE OPERATIONAL:

  •  Guide residents to meet their spiritual, emotional, physical and social needs by following an individual development plan established by Wholistic Transformations CEO.


  • Demonstrate leadership by developing and sustaining relations with ministry teams, community partners, potential employers, and workforce development programs.


  • Maintain comprehensive manual and computer-based records, databases and files including individual client case files to provide data for any funding requirements with strict confidentiality in accordance with agency policies.


  • Use program resources and assessment tools to help youth identify their personality traits, skills, interests and values as they relate to exploring career options.


  • Equip youth with community resources and programs to guide them in discovering their full potential in regards to employability, community involvement, spiritual and emotional growth.


  • Develop an Individual Development Plan (IDP) with each resident that includes goals in the following life domains: career, education, employment, independent living focused on community involvement and personal development.


  • Provide ongoing support to youth to encourage achievement of goals and connect them to services that offer opportunities and/or remove barriers.


  • Advocate for youth’s educational rights and respond to requests for information.


  • Help youth navigate and develop their own solutions, resources and solve their own problems through Community Health Evangelism (CHE) training programs.


  • Ensure the safety of all youth residing in the Wholistic Transformation community by upholding house rules established by the CEO and Board.


  • Collaborate with the CEO and Board to evaluate needs of the program and the ongoing development of the overall program.


  • Supervise the general security of the property and report any acts of inappropriate conduct, destruction or theft of property utilizing an incident report.


  • Teach the residents to take pride in the community by keeping the site free and clear of debris as well as maintaining the home units and surrounding area.

QUALIFICATIONS AND EXPERIENCE:

  •  Bachelor’s degree in social services or related field or a minimum of 4 years of experience navigating community sustainability programs and removing barriers for disadvantaged individuals, Missionary Services or Workforce and Career Development.


  • Minimum of 2 years of previous experience serving youth preferred.


  • Must be willing to reside on property with a commitment to serve in this role a minimum of one year to create stability for the residents.


  • Must have a Level 1 fingerprint clearance card or the ability to acquire one at the time of hire through the term of agreement.


  • Commitment to Wholistic Transformation Tucson’s mission and goals with the ability to relate and work with a wide range of diverse backgrounds and community organizations.
